Care Coordination
Our large and trusted network means we can help you source quality, reliable people that best match your needs – including professionals to help around the home, allied health providers, clinical carers, or suppliers to make your home safe and easy to navigate.
Of course, you may prefer to self-manage and source your own people to care and support you. Either way, we can assist by undertaking background checks on carers and maintaining communication and reviews of their service.

For Care has extensive experience providing community support.
We offer a caring, helping hand at home and in the community – from help with domestic tasks, to personal care, yard maintenance, and assistance with shopping, appointments, and staying in touch with the activities you love.

Eligibility

Apply for Assessment
Apply for an Assessment, with the government’s Aged Care Assessment Team (ACAT) online myagedcare.gov.au/assessment/apply-online or call 1800 200 422.
Online Assessment
Begin with a 20-minute online assessment
In-Home Assessment
You’ll be contacted within 2-6 weeks to arrange a more thorough assessment, which will be undertaken in your home
Application Update
You’ll receive a My Aged Care welcome pack in the mail containing helpful information and outlining what your next steps will be.
You can track your application by logging in to your My Aged Care Online Account or calling 1800 200 422.
Approval
When approved, you’ll be added to the waiting list to receive a Home Care Package based on one of four levels.
Waiting times can vary, up to 12 months, depending on your needs and place on the priority list. (If you face a long wait you may be given access to the Commonwealth Home Support Program in the interim.
Choose a Provider
You can now select a Provider to manage your Home Care funds and develop your care Plan
Low Care Needs
You may be given access to the Commonwealth Home Support Program (CHSP) if the
assessment determines you only need a low level of care.

Higher Levels of Care
If you’re eligible for higher levels of care, ACAT will arrange the next stage of assessment.
Self-Managed Care Coordination
You can choose to take the time to find your care team and brief them on your needs.
Self-Managed Delivery of Services
Organise with your care team starting dates and contracts, if relevant.
