Care and Support Services
You retain choice and control over the way your service is set up: whether you want to be hands-on with the coordination of your services, or you’d like to leave it to us to look after the day-today planning. Either way, we can assist by undertaking background checks on carers and maintaining communication and reviews of their service.

We’ll help you find the perfect match through our expert support team, as well as our large and trusted network of healthcare and service providers.
We’ll help source quality, reliable people that best align with your needs – from domestic help around the home, to allied health providers, clinical carers, or suppliers to make your home safe and easy to navigate. Our goal is to help you stay in touch with the things you love, and receive services in the place where you feel safe and comfortable.
Day-to-Day Administration
We’ll provide peace of mind by managing the financial administration of your fund on your behalf, meeting Government compliance requirements, and directly invoice Services Australia – the Government body responsible for holding your Support at Home funding in your name. We’ll laiise with your support team, pay bills and ensure you are getting the most from your funding.
A Personalised Care Plan
We work with you to create a personalised care plan that best supports your health and wellbeing goals and aspirations. We’ll regularly assess your goals, needs and preferences to ensure your services are always aligned with everything you need. Whether you choose to self-manage your services, or prefer the support of our trusted care coordination team, we’ll make sure your care always meets the highest standards..

Eligibility
Our services are delivered with the help of the Australian Government-funded Support at Home program. This funding can help pay for your aged care service, or contribute towards its cost, helping you live independently and comfortably at home for as long as possible.
If you are over 65 years of age (or 50 years if you are Aboriginal or Torres Islands), and an Australian citizen, you may be eligible for a Support at Home Package.

Apply for Assessment
Support at Home offers a single assessment system.
The online application is quick and easy, and will only take 15 to 20 minutes to complete, using your Medicare card. Youc an find it here myagedcare.gov.au/assessment/apply-online or you may prefer to call 1800 200 422.
In-Home Assessment
If your application is successful, an assessor will contact you to arrange an in-home assessment, to determine exactly what support you need to help you return to, or keep, the level of independence needed to manage your day-to-day life.
Application Update
You’ll receive a My Aged Care welcome pack in the mail containing helpful information and outlining what your next steps will be.

Approval
When approved, you’ll be added to the waiting list to receive Support at Home funding, based on one of eight classifications. Your aaplication approval will also include your quarterly budget, and any approved short-term supports.
Waiting times can vary, depending on your needs and place on the priority list. While wait lists have been up to 12 months until recently, the government is aiming to achieve shorter wait times of three months by July 2027. It has also committed to releasing 83,000 Support at Home (SAH) packages beginning in November 2025, with the remaining 43,000 scheduled for the first half of 2026.
Choose a Provider
It’s a good idea to start looking for a registered aged care provider while you are waiting on your funding, so you are ready to receive care and support straight away.
Once you’ve been given access to the Support at Home program, it continues for as long as you need home care services.
Funding approved on or before 12 September 2024
If your Home Care Package was approved before this date, your transition to Support at Home is automatic. You’ll move to the new program under the Support at Home transition rules with the same level of funding and no need for reassessment. These are referred to as “grandfathered” Home Care Packages, with protections in place to ensure indiviudals are no worse off in the transition.
Your annual funding will shift to a quarterly budget, giving you a clearer view of how your money is allocated and spent throughout the year.
Any unspent funds you currently hold will carry over. However, under the new system, future unspent funds will be capped at 10% (or $1,000, whichever is higher) to promote more consistent use of care budgets.
Funding approved after 12 September 2024
If your Home Care Package approval occured after this date, you’ll be assessed under the new Support at Home 2025 model from the start. Your Support at Home contributions and care levels will reflect the updated structure.
You’ll begin with a quarterly budget cycle, offering immediate transparency and flexibility.
Unspent funds will be capped at 10%, but any funds accrued before 1 November 2025 will transition across to your new Support at Home account.
