Just three steps
Changing aged care providers is far simpler than you might expect. For Care can look after the coordination, paperwork and continuity of care, so you don’t have to figure it out alone.
1

It begins with a few questions
We take the time to understand your situation – what’s working, what isn’t and what better support could look like for you or your loved one.
2

We build your support plan
We learn about your routines, preferences, health needs and goals. Then we design care in collaboration with you around the way you want to live at home.
3

We manage the transition for you
From paperwork to provider communication to ensuring nothing falls through the cracks – our team coordinates the changeover on your behalf.
Support that feels right
With consistent carers, clear communication and a team that genuinely listens and cares, you’ll feel the difference from your very first visit. Whether you’re receiving:

Transitioning into the new Support at Home system
For Care can help make the process simple, supportive and low stress.

Relationship-led support
We prioritise genuine relationships, trust and understanding – not just schedules and checklists. Taking the time to genuinely know the people we support helps create better experiences and better outcomes.

Experience with complex needs
Our team has experience supporting individuals with a wide range of physical, behavioural and psychosocial support needs, always with compassion, professionalism and respect.

Consistency builds confidence
Familiar faces matter. Consistent support builds trust, comfort and reassurance for both clients and families, especially when navigating change or complex needs.

Open and responsive communication
Families should never feel left in the dark. We value transparency, responsiveness and genuine connection, so people always know they have someone they can rely on.

Thoughtful support matching
We carefully consider personalities, routines and preferences when matching support workers, creating more comfortable and positive support experiences.

Flexible and personalised care
No two people are the same. Our support adapts around the individual – not the other way around.

A genuine focus on quality of life
We support people to maintain independence, build confidence and stay connected through meaningful everyday experiences and relationships.

Clinical care and a deep understanding
We combine professional knowledge with practical experience and genuine care to provide safe, informed and person-centred support.
A different approach to Aged Care
From the moment services begin, our focus is helping people feel comfortable, confident and genuinely cared for. Because the little things matter.
Quality care begins with truly understanding the person for the individual they are. The routines that bring comfort. The way someone likes their mornings. What helps them feel independent, connected and at ease in their own home. We prioritise responsiveness, transparency, genuine connection and providing thoughtful care that improves quality of life for both clients and families.
Our comprehensive services cover everything from personal care and domestic assistance to nursing, dementia support, and community transport.
Frequently Asked Questions
Can I change my current aged care provider?
Yes – you have the right to choose a provider that better suits your needs, preferences and lifestyle. If your current support no longer feels right, you are not locked in.
Will I lose my funding if I switch?
No. Your aged care funding stays with you, not with your provider. Changing providers does not affect your entitlements.
Is the process difficult or disruptive?
It doesn’t have to be. With the right guidance, most transitions are smoother than people expect. At For Care, we can coordinate the changeover – including communication with your current provider – so you don’t have to manage it alone.
How long does switching take?
This depends on your current service agreement and circumstances, but many transitions come together relatively quickly. We’ll give you a clear picture from the start.
Can I keep my existing allied health team?
In many cases, yes. If you’ve already found an OT, physio or podiatrist who knows you and your body, it makes sense to keep that relationship. We’ll work around your existing team wherever possible – and if you don’t have one yet, we have a wide network of verified allied health professionals to connect you with.
Can you help me understand the new Support at Home program?
Absolutely. We help clients and families understand their classification, what they’re entitled to, and how to make the most of their supports under the new system.
