Understanding You
We begin by getting to know you as a person. Together, we’ll discuss your goals, strengths, preferences, health, living arrangements, culture, and the outcomes you want to achieve. We also welcome the involvement of your family, carers, or chosen representatives if you would like them to participate.
Developing Your Personalised Care Plan
Working in partnership with you, we’ll develop a Care Plan that reflects your choices, respects your rights, and supports your independence. Your plan outlines the services you choose, your personal goals, and how your Support at Home funding will be used to achieve the outcomes that are important to you.
Clinical and Professional Support
Where additional clinical advice is needed, our experienced nurses and allied health professionals work collaboratively with your Care Partner, GP, and other health professionals to ensure your care is safe, coordinated, evidence-based, and responsive to your changing needs.
Choice, Control and Care Coordination
You remain in control of your care. We provide clear information about your support options, help you make informed decisions, and coordinate services that reflect your preferences, routines, cultural needs, and lifestyle. Your services can be adjusted whenever your needs or goals change.
