With the introduction of the Support at Home program starting 1 November 2025, a dynamic new option has emerged: the Restorative Care Pathway.  

It’s a powerful, short-term, goal-focused initiative that supports recovery and independence after setbacks like illness, surgery, or functional decline. 

What the Restorative Care Pathway Offers

  • Time-limited support of typically 12 weeks, extendable to 16 weeks when clinically justified. 
  • Access to a budget of around $6,000, with potential to increase to $12,000 if additional clinical services are needed. 
  • Multidisciplinary, goal-oriented care, involving allied health and nursing professionals – such as physiotherapists, occupational therapists, exercise physiologists, speech pathologists, and dieticians, coordinated by a Restorative Care Partner. 
Why it Matters 

Designed to help people regain mobility, balance, confidence, and functional ability, it often bridges the gap between hospital and home life. Whether you’ve experienced a fall, surgery, illness, or general decline in ability, this approach helps you reclaim your capabilities- with measurable progress and meaningful outcomes. 

Access, Transition & Continuity  
  • To be eligible, you must be part of the Support at Home program and show potential to improve functional ability with short-term allied health support. 
  • If you’re already receiving Short-Term Restorative Care (STRC), your provider will discuss the pathway at your upcoming plan transition. 
  • The pathway includes a structured exit strategy, ensuring a smooth transition into ongoing Support at Home services – or an updated care plan if further support is required. 
  • This approach is backed by new clinical guidelines developed with input from clinicians, older Australians, and researchers to ensure consistent, high-quality delivery.
