Clinical
Care services, including nursing and allied health, can be delivered to you in your home where you feel safe and comfortable. This reduces the need for hospital stays or moving into residential aged care. Services in the Clinical category are fully covered by Support at Home funding, with no participant contribution required.

Short Term Pathways
Restorative Care Pathway
Receive additional clinically focused care for up to 16 weeks eg while recovering from surgery. This can include multidisciplinary care.
Palliative Care Pathway
Additional support if diagnosed with less than three months to live, to enable you to stay comfortably at home while receiving care.
